Tom Hallett is a physical therapist and founder of PT Link Physical Therapy. PT Link Physical Therapy was established in 2011 and has become the leading physical therapy company across Northwest Ohio and Southeast Michigan with its expansion to over 15 locations. The brand represents excellence when partnering with diverse therapists who provide accuracy in their treatment plans and passion for their patients. Tom earned his Bachelor's degree in Community Health and Wellness with an emphasis on Psychology from the University of Findlay. He went on to receive his Masters degree in physical therapy 2 years later while working full time. After receiving his Masters degree, Tom earned a Doctorate of Physical Therapy from Temple University in Philadelphia. Temple was ranked the number one physical therapy program at that time. With over 21 years of experience in physical rehabilitation and sports medicine, Tom has dedicated his career to staying at the forefront of research, education, and outcome-driven methods within the field of physical therapy. He has obtained advanced certifications in mechanical diagnosis, advanced manipulation, biomechanical dry needling, integrative dry needling, kinesio taping, and functional movement systems.

Mike Coulter

Mike Coulter, DPT

Mike Coulter joined the PT Link team in 2003. He received his Physical Therapy degree from the University of Toledo Medical College in 1992. He is also certified in the McKenzie Method, dry needling, kinesio taping, RockBlade, functional capacity evaluation, and is an ergonomic evaluation specialist. Mike loves working with people and wants to help them get back to a pain-free life. In his freetime, he enjoys volunteering as a physical therapist at community events, hanging out with his family, playing guitar, and cheering on Michigan Football.

Samuel J. Rakyta II

Samuel Rakyta II, DPT

Sam has been a Licensed Physical Therapist since 2009 and joined our PT Link team in 2014. He holds both a Doctorate in Physical Therapy (2009) and a Bachelor of Science in Exercise Science (2006) from The University of Toledo. During his time at The University of Toledo he developed a Clinical Practice Guideline for Upper Extremity Musculoskeletal Disorders and contributed to research pertaining to the influence of inflammation on muscle hypertrophy which was published in the American Journal of Physiology and Cell Physiology.

Sam is a NSCA Certified Strength and Conditioning Specialist and Human Movement Specialist through the Brookbush Institute. He has earned multiple certifications in Dry Needling, Ultrasound Guided Dry Needling, Blood Flow Restriction and High Velocity Low Amplitude Manipulation. He also has completed the ImPACT concussion training workshop and is an APTA Credentialed Clinical Instructor.

Clinically, Sam focuses on helping patients move better to live a pain-free life. He uses a wide variety of manual therapy techniques and corrective functional exercise to reduce common movement and posture dysfunctions that lead to pain. Sam also has extensive experience in sports medicine, orthopedics and working with patients that suffer from neck and back pain.

Sam is married to Holly, who is a PTA also at PT Link, and has two small children. In his free time; Sam enjoys spending time with his family and friends, exercising, hunting with his dad and is an avid fan of the Pittsburgh Steelers and Ohio State Buckeyes.

Chris Lovejoy

Chris Lovejoy, DPT

Chris Lovejoy joined the PT Link team in Waterville in 2016. He graduated from Miami University with a Bachelor of Science degree in 2006, and three years later earned his Doctorate of Physical Therapy from the University of Toledo in 2009. He also has a certification in dry needling. After rupturing his ACL playing basketball in highschool, Chris underwent surgery and physical therapy, and decided that was the career he wanted to pursue. Outside of work, he loves spending time with his family by coaching his children in sports.

Mike Hofener

Mike Hofener joined the PT Link team in 2017. He earned his Bachelor’s in Exercise Science in 2013, and later received his Doctor of Physical Therapy degree in 2017. He also has certifications in dry needling, strength and conditioning specialist, and fall prevention specialist. Mike began his physical therapy career in athletic training and loved doing the rehab side of the job, and wanted to fully help people in pain after surgeries. Outside of work, he volunteers at community care clinics, enjoys spending time with his family, and golfing.

Craig Obrock joined the PT Link team in 2020. He earned his Bachelor’s of Applied Health Science in 2012, and later received his Masters in Business Administration in 2016. He completed his education with a Doctorate of Physical Therapy in 2017. Craig is a member of the American Physical Therapy Association, and also certified as a strength and conditioning specialist, as well as dry needling, blood flow restriction, and an emergency medical responder. He pursued a career in physical therapy because he loves helping people with their strength and fitness. During his free time, Craig enjoys going on walks with his dogs and volunteering for his nephew’s baseball team.

Kayla Plaunt, DPT

Kayla Plaunt joined the PT Link team in 2021. She earned her Bachelor’s degree in Kinesiology and Exercise Science in 2012 from the University of Toledo, her Associate’s degree in Physical Therapy Assistance from the Professional Skills Institute in 2016, and her Doctorate in Physical Therapy from the University of Findlay in 2020. Kayla has certifications in kinesio taping, mobilization of the cervical and thoracic spine and ribs, dry needling, Level 1 of SFMA and FMA, and manual therapy of the lower quadrant. She chose to pursue physical therapy as a career, because of her interests in human anatomy, exercising, and helping people. During her freetime, she volunteers at the Ability Center and Special Olympics, and also enjoys exercising, being outdoors, and spending time with her dog.

Matthew Milligan

Matthew Milligan joined the PT Link team in 2020. He earned his Bachelor of Science in Applied Health Science from Bowling Green State University in 2014, and later received his Doctor of Physical Therapy degree from the University of Toledo in 2017. John Longmore joined the PT Link team in 2013 graduated from University of Toledo with a Bachelor of Science in 1988, and later earned his Associate of Applied Sciences from Professional Skills Institute in 1995. He is also certified in kineseo taping, HVLA, and Mulligan techniques. John became interested in physical therapy because he wanted to work with people and get them back to what they enjoy doing with little to no pain. He loves spending time with his family, and loved coaching basketball in his freetime.

Makenzie Kretz graduated from Professional Skills Institute in 2020 with an Associates Degree, and joined PT Link in 2021. She began pursuing physical therapy because she enjoys helping people, seeing them making progress, and reach their goals. When she’s not at work, her favorite pastimes are drawing, painting, and taking her dogs to the park.

She earned her Physical Therapy Assistant degree from Edison State Community College in 2021, and is certified as a personal trainer. Mariah began a career in physical therapy, because she had gone through physical therapy herself and wanted to help other people in the same way her therapist helped her. In her freetime, she volunteers for the kids ministry at her church, and enjoys hiking, kayaking, and painting.

She earned her associates degree from Professional Skills Institute in physical therapy assistance, and later went on to receive her Bachelors in Exercise Science from the University of Toledo. Erin started her career in physical therapy, because she went through a lot of physical therapy as a child to treat her chronic back and hip issues, and wanted to do the same for others. She is certified in blade rock, cupping, kinesio taping, and the Mulligan (mobilization with movement) technique. In her freetime, she loves to spend time with her family and dogs, as well as running marathons, and biking.

Amelia Smith

Amelia Smith joined PT Link in 2022. She earned her Associates of Science and General Studies at Monroe Community College in 2014, and three years later in 2017, she received her Associate of Applied Science in Physical Therapy Assistance from Professional Skills Institute. Amelia grew up attending physical therapy sessions due to developmental issues, and wanted to work in the medical field without being in a hospital. When she’s not at work, she loves spending time with her husband and son, and bartending at a banquet hall.

Konnor Chuba

Konnor joined the PT Link Team in Maumee in 2021. He earned his Bachelor of Science Degree from the University of Toledo in 2015, and later received his Associates Degree in Physical Therapy Assistance from Professional Skills Institute in 2021. He was first introduced to physical therapy when he was younger and became interested in the process of physical rehabilitation. When he’s not at work, Konnor loves playing with his daughter and watching football.

Jared Rickle earned his Associates for Physical Therapy Assistance from Rhodes State College in 2017, and joined PT Link in 2022. He took an interest in physical therapy when he experienced a wrist injury playing high school football. After attending physical therapy sessions to play again, he decided he wanted to help people recover from their injuries to get back to what they love doing. Jared is certified in physical therapy of the fitness athlete, pain science treatment, and BIG rehabilitation for Parkinson’s disease. Outside of work, he helps family members and neighbors with yard work if they are unable to do so. Jared also enjoys cheering on the Cleveland Browns and Ohio State Buckeyes, playing golf, hunting, and fishing.

Physical therapy advice: “Be patient with your rehab and always keep pushing forward. Attitude and optimism are key to achieving a full recovery.”

Nick Skotynsky joined PT Link in 2018 and is the Clinic Director at the Meijer Drive location in Toledo. He also received his Associates Degree for Applied Sciences from Professional Skills Institute in 2018. Nick is certified in the Mulligan (mobilization with movement) technique. He has a passion for fitness and helping people in his community, and physical therapy was a field in which he could pursue both interests. During his freetime, he is a volunteer firefighter for the Middleton Township Fire Department, and enjoys spending time with his family.

Rayne Kohlenberg graduated from Terra State Community College in 2022 with an Associates in Applied Science with a specialization in Physical Therapy Assistance, and joined PT Link later that year. She is also a State Tested Nursing Assistant (STNA). Rayne first became interested in physical therapy because she wanted to help people, and saw her family and friends go through physical therapy with positive outcomes. In her freetime, she enjoys teaching her children to cook and playing outside with them.

Leah Taylor joined the PT Link team in 2022. She graduated from Professional Skills Institute with an Associates degree in Physical Therapy Assistance in 2016, and has a certification in Parkinson Wellness Recovery. Leah took an interest in physical therapy when a family member was impacted by an invasive surgery that she says could have been prevented by going to physical therapy. She helped them get to post-surgery appointments, and loved the idea of being able to show people a wider range of alternative recovery options to surgery. Outside of work, Leah loves to read and spend time with her daughter.

Physical therapy advice: “Speak up. You can not give us too much information and what you think we might not care to hear about may be important to your progress and improvement.”
